On his New Zealand debut, it took McIntosh 38 deliveries to score his first runs in Tests. However, he impressed in his second Test, scoring a patient 136, his maiden Test century, to help New Zealand secure a draw against the West Indies.

He is a native of Auckland, Auckland Region, New Zealand as he was born and spent most of his early life there. Tim McIntosh earned a call-up to the New Zealand Test squad at the age of 29, to face the touring West Indians. He made his debut for Auckland in 1998-99 but got the chance to play only a handful of matches in his first two seasons. His consistency stood out in the next three seasons for the Aces and in 2002-03, amassed 820 runs at 58.57. He had a forgettable season with Canterbury in 2004-05 but ironically, his highest first-class score of 268 came against them. He had one unsuccessful season for the Canterbury Wizards averaging only 4.90. A sound start to the 2008-09 season caught the eye of the selectors who were looking to revamp New Zealand's struggling top order. After a edgy debut Test in Dunedin, he hit a patient century in the following game in Napier. The 2010 tour of India started disastrously for McIntosh, when he scored a pair in the first Test at Ahmedabad. However, he bounced back and scored a patient 102 in the next Test at Hyderabad, becoming only the 11th batsman in the game to score a century after getting a pair in the previous match. McIntosh is largely regarded as a four-day specialist, a graduate of the 'Mark Richardson School of Batting.'
